James Madison Middle School

A VHSL Sport Physical is NOT the same thing as an annual physical for school! VHSL Physical must not be dated any earlier than May 1^st for the upcoming school year (ie May 1^st, 2023 for 2023-24 school year).

 

A physical dated prior to May 1^st cannot carry over to the next school year.

You must have a VHSL form on file if you plan on participating in ANY sport (including tryouts)!

If you are a middle school student and you play a sport at the high school level, you MUST get two copies of your VHSL - one for the high school, and one for the middle school.
